We utilize Community Health Workers to assist patients with their social needs, including using uber for transportation as an example.

Does your organization screen or try to identify if your patients have un met social needs - housing, food or transportation insecurity??

Yes

How about what do you do when a patient says I can't get to my appointment because I don't have transportation?

The clinic staff assist the patient in getting transportation (public transport or uber)

Not sure what you are wanting to see so but we are in an indigent area so our CHWs spend the majority of time helping with transportation and getting people safe housing. our CHWs spend a lot of time teaching patients how to use public transportation as well

Yes. Annual Care Management screening for Social Determinants of Health and possible Care Management outreach - home visits, social workers, transportation support, community resources, etc. Clinical staff at each office help with social needs as they come up as well.

We have the forms to collect the data, I just don't have the information about all the things the CHW's are doing
